Hospice Care Program

Support through the Hospice Care Program is offered through a personalized plan of care to ease the many different types of pain facing patients and families during such a difficult time. The Hospice Care Team consists of the medical director, registered nurses, social worker, spiritual counselor, trained volunteers, home health aide, and a Bereavement/Volunteer Coordinator.  The Team assists our patients live their lives comfortably without pain while managing the symptoms of the illness.  Extensive assistance, information and education are provided to the patient’s families.  Support is available 24 hours a day, 7 days a week.
